Intake and referral

Referrals will be received directly by the Resilience Staff from various public and private sources. Admission criteria are reviewed at the time of each referral to determine appropriateness of level of care. Consumers experiencing a mental health emergency and in need of immediate services will be required to call 911 or go to the nearest emergency hospital.

Admission and Consent

Upon the initial clinical appointment, the consumer will complete Intake paperwork, which includes:

Consumer Civil Rights Compliance

Complaint and Grievance Procedure

Benefit/Eligibility Coverage

Payment for Services/Cancellation Policy

Consent for Release of Information

Attendance Policy

Billing and Signature Policy

The Psychiatrist/Practitioner will discuss the following information with the consumer or guardian in order to ensure that the consumer or guardian is being fully and accurately informed about the consumer’s treatment and that the consumer or guardian is able to give Informed Consent.

The level of care proposed for the consumer.

The service options.

Explanation of proposed services.

Possible problems associated with that type of service.

Potential benefits of the service.

Possible side effects and risks of the therapeutic service.

The right to refuse treatment.
